Hebephilia as Mental Disorder? A Historical, Cross-Cultural, Sociological, Cross-Species, Non-Clinical Empirical, and Evolutionary Review

Abstract

Blanchard et al. (2009) demonstrated that hebephilia is a genuine sexual preference, but then proposed, without argument or evidence, that it should be designated as a mental disorder in the DSM-5. A series of Letters-to-the-Editor criticized this proposal as a non sequitur. Blanchard (2009), in rebuttal, reaffirmed his position, but without adequately addressing some central criticisms. In this article, we examine hebephilia-as-disorder in full detail. Unlike Blanchard et al., we discuss definitions of mental disorder, examine extensive evidence from a broad range of sources, and consider alternative (i.e., non-pathological) explanations for hebephilia. We employed Wakefield’s (1992b) harmful dysfunction approach to disorder, which holds that a condition only counts as a disorder when it is a failure of a naturally selected mechanism to function as designed, which is harmful to the individual in the current environment. We also considered a harmful-for-others approach to disorder (Brülde, 2007). Examination of historical, cross-cultural, sociological, cross-species, non-clinical empirical, and evolutionary evidence and perspectives indicated that hebephilic interest is an evolved capacity and hebephilic preference an expectable distributional variant, both of which were adaptively neutral or functional, not dysfunctional, in earlier human environments. Hebephilia’s conflict with modern society makes it an evolutionary mismatch, not a genuine disorder. Though it should not be classified as a disorder, it could be entered in the DSM’s 5-code section, used for non-disordered conditions that create significant problems in present-day society.

A normal range for testosterone is between 280 to 1,100 nanograms per deciliter (ng/dl). However, low testosterone in men is considered to be below 300 ng/dl. When a man has a low level of testosterone, it may be referred to as low testosterone, low T, hypogonadism and/or testosterone deficiency.

Would a man necessarily know if he his testosterone levels are low? And if they are, why does it matter? Men with low T may have several bells and whistles trying to get his attention that low T is his problem. From his sex life suffering to having certain health parameters out of range such as high blood pressure and high cholesterol, these can be signals something within is not right and is affecting his health and well-being.

Men who suspect that low testosterone might be the trigger for certain symptoms he is experiencing, need to be familiar with signs of low T. Ignoring these signs or symptoms is not advised.

It is important for a man to discuss these symptoms with his doctor and to get his testosterone levels checked. If it is low T, it can be replaced to make up for what his body is no longer producing enough of. Just like blood pressure or thyroid levels are treated to help bring back to a normal state, low T needs the same attention. Not addressing a testosterone decline can subject men to an increased risk for bone fractures, diabetes, and heart disease, as well as cognitive declines, loss of sexual performance, and overall lack of motivation.

Low libido

One of the most significant and first signs of low T is a reduced interest in sex. Some men may chalk it up to getting older, as it can be common for sex drive to decline with age. But men with low T will usually have a noticeable drop in their desire for sex.

Erectile dysfunction

Testosterone is the driver turning on the engine for sexual desire, but it also is responsible for helping a man achieve and maintain an erection. Testosterone works together with nitric oxide, a molecule triggering a series of chemical reactions that is necessary for an erection to occur. If testosterone levels plummet, a man will have difficulty in achieving an erection sufficient for sexual intercourse.

Fatigue

When testosterone levels drop, this can result in a drop in a man’s emotional well-being and an increased likelihood of depression and moodiness. For many men, these types of emotional shifts can be some of the first indications of low T. Research has shown that up to 56% of men with low T will also have significant symptoms of depression.

Decreased bone mass

Even though the brittle bone disease of osteoporosis is mainly associated with women, men with low T can also experience thinning bones. Testosterone helps produce and strengthen bone and when levels are below normal this means a man may have lower bone volume making them more susceptible to bone fractures.

Loss of muscle mass

What helps play a role in giving men their muscular physique is the hormone testosterone. If a man is noticing his muscle mass is less than usual, he might be able to blame it on low T. Studies have shown testosterone affects muscle mass, but not necessarily strength or function.

Breast growth and increased body fat

Low testosterone levels in men can sometimes lead to increased body fat and a condition called gynecomastia, or the development of larger breasts. The male body produces both testosterone and estrogen, although estrogen is usually found at low levels. But if a man’s testosterone levels are especially low in comparison to estrogen, or if there is an excess of estrogen relative to testosterone, larger breast may develop along with more body fat leading to extra weight gain.

Changes in sleep patterns

In some men, low testosterone can cause insomnia or other sleep disturbances.

Trouble concentrating

Many men with low T complain of “brain fog” or find themselves getting off track easily due to trouble concentrating. Memory loss is another common complaint of men with low T that has started to affect their daily life.

By getting tested and then treated for low T, this can help a man avoid many of the health issues associated with this common condition and to have better management over his health and well-being.

Writing for Cosmetic Surgery Times, dermatologic surgeon Jason Emer, M.D. explained: 'As the vaginal rejuvenation market is skyrocketing, men are seeking their own type of rejuvenation. Who wouldn’t want to be a little bit longer, thicker, or have more sensitivity and a better sex life? These men are also becoming interested in the cosmetic appearance of the actual penis and scrotum itself.'

It's perhaps unsurprising that men are investing in quirky treatments after research revealed that the number of men having cosmetic surgery has doubled over the past decade.

Albania Rejects Sentence Cut for British Paedophile

A judge has rejected a request to cut the sentence given to a British man found guilty of sexually abusing children at an orphanage in Tirana because he still denies the charge.

The judge rejected the request for a sentence reduction on grounds of good behaviour from Robin Arnold, who was jailed in 2010, along with two other British nationals, for abusing and raping scores of young children in a Tirana orphanage, founded by one of the defendants.

Arnold, who was jailed for 15 years for having sexual intercourse with minors, had sought a 90-day reduction in his sentence. He received a similar reduction in 2010, but this time Tirana judge Marjola Xhaka noted that Arnold had yet to show remorse for what he had done.

“Considering the fact the defendant denies committing the criminal act, in light of the nature of this kind of criminal act, the court believes that the defendant is not showing proof of his rehabilitation and the possibility of reintegration into society,” Xhaka wrote in the verdict.

According to Albanian law, a convict can seek a 90-day reduction of their sentence every year, based on good behaviour in prison and proof that they have tried to rehabilitate themselves.

Arnold was convicted of having been part of a paedophile ring which also included two other British nationals, Dino Christodoulou and David Brown, who were both sentenced to 20 years in prison. The three defendants were found guilty of abusing ten Roma children between the ages of four and 14 at the His Children orphanage in Tirana, which was founded in 2001 by Brown, an evangelical Christian charity worker.

Arnold and Christodoulou started visiting the orphanage in 2002, where they had unsupervised access to the children, the trial heard. Acting on a tip-off, police raided the orphanage in May 2006, arresting Brown, while Christodoulou and Arnold were extradited to Albania later.

An investigation by The Guardian newspaper later revealed that Britain's National Criminal Intelligence Service, NCIS, received details about the abuse at the children’s home 18 months earlier, in December 2004, but failed to tell its Albanian counterparts. Arnold still has 12 years of imprisonment to serve.

Pedophilia in Serbia

According to experts, one in three girls and one in seven boys fall victim to pedophiles worldwide. Serbia is no different.

There is a discrepancy between the number of reported cases and the number of sexually abused children. The estimate is that two-thirds of children in Serbia have been abused. Pedophilia cases are typically brought into public attention only with drastic crimes or when they involve a well-known public figure.

Most of the time, however, the aggressors remain anonymous to the majority of citizens, but also to their immediate surroundings.

The molesters are most often people whom the child knows: relatives and neighbors. Still, the abuse cases get reported most likely if the perpetrator is unknown. The process starts with seducing the child, and ends with threats about someone close to the child being hurt. In as many as 89,75 per cent of the cases, the abuse goes on for several years, and in 10,25 per cent for several months. Milena Živanovic of the Belgrade MUP says that the aggressors manipulate the children easily, exploiting their trust and love, and often using fear and emotional blackmail to stop the child from speaking. When they finally do, the molesters lose status and reputation, and can easily end up in prison. For this reason, they are ready for anything in order to stop the children from revealing their ordeal.

Milena Živanovic told the Vecernje novosti daily that it is of utmost importance to believe the child when it gathers enough courage to speak, and only after that investigate whether what it had to say is true. “Can you even start to imagine what it must be like for the child to tell you what it’s been through, and your reaction is that it’s made up? That’s like killing the child on the spot”, she says.

The verdicts will not yield results if the courts go for the legal minimum, Milena Živanovic says and adds that there is almost one hundred per cent chance of repeating the offence among that category of criminal. “New measures for the protection of the child are needed. Abroad, convicted pedophiles are banned from entering facilities with children; they are fitted with tracking devices, etc. Still, the first steps in combating the abuse include creating a register and a team approach to the problem. If we were to create the central register, available, under defined circumstances, to expert services involved in child care, such as kindergartens, schools, social services centers, and others, we could prevent pedophiles from being employed at such institutions”, Milena Živanovic says.

Vesna Brzev-Curcic, a therapist, says that sexual abuse is no longer a taboo and that the children are learning about their rights and are encouraged to bring their problems up. “Sexual abuse creates dramatic consequences and depends on the child’s age. Children are prone to withdrawing from society, alcoholism, prostitution and drug abuse. Abused children often become abusers themselves, or lifelong victims. The victimized children are best helped if they are believed. That faith in them enables them to trust people again, alleviate the guilt they feel, and the shame. Another aspect is the preventive behavior of the society as a whole, especially the judicial system. An early detection system needs to be developed, sentences should be harsher, and the support services strengthened”, Vesna Brzev-Curcic says.

Changes to the laws seek to equate the rape of a child to any similar crime. The sentences range from three to 18 years in prison.

In China, a woman can be imported from North Korea for about $1500, less than the price of a decent used car. And the business of trafficking women from North Korea to China is booming.

Korea estimates that anywhere from 50,000 to 100,000 people from North Korea are currently living in China. Of those, about 80% are women. And of the women, almost 90% have been trafficked at some point or are currently victims of trafficking. For the most part, these women are trafficked into marriages. The one child policy in China has created a generation where men greatly outnumber women and Chinese wives are hard to come by. So Chinese men who want to get married are forced to look elsewhere. Some try to meet foreign women through legitimate means. But others just buy a wife from North Korea and have her shipped over, like an imported wine. Or rather, a piece of imported meat.

Sex trafficking is also going strong, serving the Chinese men who are looking to get laid rather than married. Sometimes, women are offered jobs in the Chinese tech industry. Those jobs turn out to be stripping for Internet webcasts and/or forced prostitution. North Korean women who are forced into prostitution face even more risks than those forced into marriages, because if caught, they face additional punishments back home.

Both the forced marriages and the sex trafficking are leading to a generation of Chinese-Korean children without a clear home. The children of trafficked women and their husbands or johns often end up not just homeless, but stateless as well. Usually, this happens when the Korean mother is caught in China without proper documentation and deported to North Korea, often to prison or a labor camp where she can’t bring the child. If the Chinese father doesn’t take responsibility for the child, then the kid ends up an orphan which no parent or country able and willing to take care of him or her.

The cross-border trafficking of women from North Korea to China has become an epidemic in the truest sense to the word. It’s spreading farther into both countries than the border region and infecting thousands of women. It’s even affecting a new generation of children, living without a family or a country to call home.

Zimbabwe: Age of Consent to Go Up to 18 - Mnangagwa

VICE PRESIDENT Emmerson Mnangagwa says government was in the process of introducing a law that would see the statutory age of consent to sex by girls raised from 16 to 18 years.

He was responding to calls by Senators on Tuesday that the minimum age one could be allowed to consent to sex should be aligned to a 2016 Constitutional Court ruling which outlawed the marriage of or among minors below 18.

While the ruling was followed with euphoric victory among child and women's rights activists, some felt it remained hollow as girls as young as 16 could still indulge in sex and even conceive children for as long as they did not proceed to get married before 18.

"We have a landmark ruling in this country which states that nobody should be married or be married off when they are below the age of 18," Senator representing people with disabilities, Anna Shiri, had said earlier.

She was contributing to a motion which called on the government to ratify and incorporate into its gender laws, the SADC Model Law on Eradicating Child Marriage.

Zimbabwe, currently, is in the process of enacting a law which seeks to operationalise the outlawing of child marriages.

"At the same time we have this Bill," Shiri said, "while we have part of the Act which says there is an age of consent to sex which is 16 years of age.

"As a result, there is some contradiction where early marriage is 18 years yet the consent to sexual activity is 16 years. We need to align these laws."

Mnangagwa, who was speaking as the country's justice minister, said government was considering raising the age of consent to 18.

"I may also say that the other two points you have raised relating to the issue of consent between juveniles or children, anybody who is below 18 is regarded as a child," Mnangagwa said.

"We had a Committee to deal with that and we have arrived at a possible solution which will come to Parliament on the issue of consent between an adult and juvenile or between a juvenile and a juvenile.

"Those issues we have debated and we believe that we should bring up also the age of consent to the age of 18. Of course, this is subject to debate when it comes to Parliament."

Mnangagwa said his ministry was in the process of aligning all the marriage laws and the progressive provisions of the SADC Model law he said will be incorporated in the comprehensive proposed Marriages Bill.

The proposed Bill will be able to amend the Marriages Act and the Customary Marriages Act and all other laws that are outdated in relation to marriages.

FGM victims to undergo free surgery

The Karen Hospital will for the next few days be offering free surgery to reconstruct the sexual organs of women who have undergone female genital mutilation.

In a drive dubbed ‘Restore Pink Plus’, FGM survivors will undergo clitoroplasty surgery, a clitoral reconstructive operation, with the aim of restoring the function of the clitoris and the women’s dignity.

Surgeons carrying out the procedure say the operation could stop pain, help women feel sexual pleasure and restore their identity and femininity.

“The initiative has been designed to assist FGM victims undergo clitoral reconstruction in order to restore their identity and dignity and rejuvenate the clitoris’s sensation,” Dr Abdullahi Adan, who is pioneering the initiative, said.

SIDE-EFFECTS Dr Adan, the Grand Round Coordinator, Department of Surgery at the University of Nairobi and plastic, aesthetic and reconstructive Surgeon at The Karen and Kenyatta National hospitals, and Dr Marci Bowers, Clitoraid’s obstetrics/gynaecology surgeon from the USA, will be the lead surgeons.

The procedure being pro-bono, patients will not be charged the doctor’s fee but will only pay for consumables.

This will be the first time such a surgery is being conducted in Kenya.

Most FGM survivors suffer painful side-effects and a loss of sexual pleasure.

COUNSELLING

While it cannot fully restore the genitalia to how it would have been had the woman not been mutilated, the surgery is meant to rebuild the damaged area for women who have undergone incision and rejuvenate the nerve networks so that they can regain sensitivity and, in some cases, attain orgasm.

The initiative also has the women go through counselling, which is vital for the emotional healing due to the traumatising effects of FGM in their early childhood.

FGM, the partial or total removal of the external genitalia for non-medical reasons, is a cultural tradition widely practiced by some Kenyan communities.

REGISTRATION ONGOING Last week, the surgeons conducted capacity building training for other Kenyan doctors on the new surgery and issued certificates to the trainees who will join the surgical team.

The week-long surgeries are set to begin tomorrow and run until May 12.

So far, 30 patients have registered for the surgery.

However, because of the high demand for the rare surgery, the doctors say they may have to extend the operations for another week.
